4. I agree to take part in the study. I understand that taking part in the study will include completing the health profile information in biomesight.com.
5. I understand that my taking part is voluntary.

2. I understand my personal details such as name, address, and email address etc. will not be revealed to people outside of Biomesight staff and contractors.
3. I understand that Biomesight may involve 3rd party collaborators in this research study and agree that my anonymised sample results and health profile information may be shared and analysed outside of Biomesight.
4. I understand and agree that Biomesight and their authorised research collaborators may use my data in anonymised, aggregated format in publications, reports, web pages, and other research outputs, only if they agree to preserve the confidentiality of the information as requested in this form.

2. I understand that if I have agreed to this optional consent, that my sample results and health profile may be shared with other research projects without obtaining additional consent.
